Overview

This is a combination regimen of three drugs used in investigational cancer therapy: - **Carboplatin** is a platinum-based chemotherapy agent that causes DNA crosslinking and damage, leading to apoptosis in rapidly dividing cells. - **Paclitaxel** is a taxane chemotherapy that stabilizes microtubules and inhibits cell division. - **Pevonedistat (MLN4924)** is an investigational small molecule inhibitor of the NEDD8-activating enzyme (NAE), which disrupts neddylation—a process essential for the degradation of proteins involved in cell cycle progression and survival. By inhibiting NAE, pevonedistat leads to accumulation of proteins that induce cell cycle arrest and apoptosis. Preclinical studies suggest synergy between pevonedistat and platinum/taxane chemotherapies by interfering with DNA repair pathways such as nucleotide excision repair (NER) and interstrand crosslink repair (ICR). This triple combination has been studied primarily in advanced solid tumors—including non-small cell lung cancer, intrahepatic cholangiocarcinoma, bile duct cancer—and has shown some evidence of activity even in patients previously resistant to platinum or taxane therapies[2][3][5][9]. However, recent phase II data indicate insufficient efficacy for further development[1].
