Overview

This drug is a combination chemotherapy regimen consisting of three agents: carboplatin, paclitaxel, and suramin. Carboplatin is a platinum-based alkylating agent that causes DNA crosslinking leading to apoptosis. Paclitaxel is a microtubule-stabilizing agent that inhibits cell division by preventing microtubule disassembly. Suramin is an experimental chemotherapeutic agent known to inhibit multiple growth factors such as acidic and basic fibroblast growth factors (aFGF and bFGF), platelet-derived growth factor (PDGF), epidermal growth factor (EGF), and tumor growth factor-beta (TGF-beta) by blocking their binding to receptors or interfering with autocrine loops involved in tumor cell proliferation. Suramin has been studied as a chemosensitizer enhancing the antitumor activity of chemotherapy drugs like paclitaxel and carboplatin without increasing toxicity significantly. The combination has been investigated primarily for advanced non-small cell lung cancer (NSCLC) stages IIIB/IV in clinical trials where low-dose suramin was used to reduce resistance to chemotherapy with paclitaxel/carboplatin. The regimen aims at synergistic effects by combining different mechanisms of action targeting tumor cells through DNA damage, mitotic arrest, and inhibition of growth factor-mediated chemoresistance pathways. Clinical studies showed promising antitumor activity with manageable side effects including neutropenia, nausea/vomiting, fatigue, peripheral neuropathy but no new toxicities beyond those expected from the individual agents. Pharmacokinetic studies helped optimize dosing of suramin to achieve plasma concentrations effective for chemosensitization while avoiding high peak levels.
