Carboplatin + pemetrexed + paclitaxel is a combination chemotherapy regimen used as the investigator's choice backbone in clinical trials for non-small cell lung cancer (NSCLC). In the context of a Phase 2 neoadjuvant study sponsored by the Medical University of South Carolina (NCT04304209), this designation refers to the administration of histology-specific platinum doublets: either carboplatin and pemetrexed (typically for non-squamous NSCLC) or carboplatin and paclitaxel (typically for squamous NSCLC). Carboplatin is a platinum-based alkylating agent that forms DNA crosslinks to inhibit replication. Pemetrexed is a folate antimetabolite that inhibits multiple enzymes in the nucleotide synthesis pathway, including thymidylate synthase (TS), dihydrofolate reductase (DHFR), and glycinamide ribonucleotide formyltransferase (GART). Paclitaxel is a taxane that stabilizes microtubules, preventing their disassembly and thereby arresting the cell cycle. This combination is being evaluated in conjunction with the PD-1 inhibitor cemiplimab for resectable Stage I NSCLC.
