Carboplatin + pemetrexed + pembrolizumab is a combination regimen used primarily as first-line therapy for advanced or metastatic non-squamous non-small cell lung cancer (NSCLC) in patients without targetable genetic alterations. Carboplatin is a platinum-based chemotherapy agent that causes DNA crosslinking and apoptosis in rapidly dividing cells. Pemetrexed is an antifolate antineoplastic agent that inhibits multiple folate-dependent enzymes involved in nucleotide synthesis, thereby blocking DNA and RNA synthesis. Pembrolizumab is a monoclonal antibody immune checkpoint inhibitor targeting the programmed death-1 (PD-1) receptor on T-cells, enhancing anti-tumor immune responses by preventing tumor-mediated T-cell inhibition. This combination has demonstrated improved survival outcomes compared to chemotherapy alone and has become standard of care for eligible NSCLC patients[1][3][6].
