Carboplatin + veliparib is a combination therapy that pairs carboplatin, a platinum-based chemotherapy agent, with veliparib, an oral poly(ADP-ribose) polymerase (PARP) inhibitor. This combination has been investigated for the treatment of various cancers, including extensive-stage small cell lung cancer (ED-SCLC) and advanced breast cancer. The PARP inhibitor veliparib works by preventing DNA repair in cancer cells, potentially enhancing the cytotoxic effects of carboplatin. The combination has been studied in clinical trials for the treatment of extensive-stage small cell lung cancer (ED-SCLC) and advanced HER2-negative breast cancer with germline BRCA1/2 mutations. In the ED-SCLC trial, the combination showed improved progression-free survival (PFS) compared to control, with a hazard ratio of 0.67, though there was no corresponding benefit in overall survival. In the BROCADE3 trial for breast cancer, the addition of veliparib to carboplatin/paclitaxel improved PFS with a hazard ratio of 0.71. Veliparib maintenance monotherapy following combination therapy demonstrated a tolerable safety profile and may extend PFS.
