Cardi-Braun Mantenimiento is a sterile, electrolyte-based cardioplegic solution developed for myocardial protection during major cardiac surgery requiring extracorporeal circulation. It is specifically formulated for the maintenance of diastolic cardiac arrest following initial induction. The solution operates through a hyperkalemic mechanism, where an elevated concentration of potassium ions depolarizes the myocardial cell membrane, preventing repolarization and effectively halting electromechanical activity. This state of diastolic arrest significantly reduces the heart's metabolic and oxygen demands during the ischemic period of surgery. Additionally, the solution contains magnesium to stabilize cell membranes and low concentrations of calcium to mitigate the risk of the calcium paradox upon reperfusion. It was developed in collaboration with clinical institutions such as Hospital Clinico San Carlos and is manufactured by B. Braun.
