The CaRD regimen is a combination therapy consisting of carfilzomib, rituximab, and dexamethasone, primarily investigated by the Dana-Farber Cancer Institute for the treatment of Waldenström macroglobulinemia (WM). Carfilzomib is a second-generation, irreversible proteasome inhibitor that prevents the degradation of proteins within cells, leading to anti-tumor activity. Rituximab is a chimeric monoclonal antibody targeting the CD20 antigen on B cells, and dexamethasone is a synthetic corticosteroid used to enhance therapeutic efficacy and manage side effects. This regimen was specifically developed as a neuropathy-sparing alternative to bortezomib-based treatments, which frequently cause severe peripheral neuropathy in WM patients. Clinical trials have demonstrated high response rates in symptomatic, treatment-naive patients, with activity that appears independent of MYD88 or CXCR4 mutation status.
