Overview

Carfilzomib + cyclophosphamide + lenalidomide + dexamethasone is a four-drug combination regimen used primarily as induction therapy for transplant-eligible, newly diagnosed multiple myeloma. This regimen, often referred to as KRdc, combines agents with distinct mechanisms of action to target myeloma cells and reduce the risk of drug resistance and relapse. Carfilzomib is a second-generation proteasome inhibitor that induces apoptosis in myeloma cells by blocking protein degradation pathways. Lenalidomide is an immunomodulatory agent that enhances immune response against tumor cells and inhibits angiogenesis. Cyclophosphamide is an alkylating agent that interferes with DNA replication in rapidly dividing cells, including cancerous plasma cells. Dexamethasone is a corticosteroid that exerts anti-inflammatory and cytotoxic effects on lymphoid malignancies[1][5]. The combination aims to maximize efficacy through synergistic targeting of multiple cellular pathways involved in myeloma pathogenesis[1].
