Overview

CAR-D PACE is an intensive, multi-agent infusional chemotherapy regimen primarily utilized for the treatment of relapsed or refractory multiple myeloma (RRMM). It is a modification of the established D-PACE regimen (dexamethasone, cisplatin, doxorubicin, cyclophosphamide, and etoposide) with the addition of the second-generation proteasome inhibitor carfilzomib. This combination is typically reserved for patients with aggressive disease or those who have progressed on multiple prior lines of therapy, including BCMA-targeted agents. The regimen leverages the synergistic effects of proteasome inhibition with traditional cytotoxic alkylating agents, topoisomerase inhibitors, and platinum-based chemotherapy to overcome drug resistance in malignant plasma cells.
