Overview

This is a combination therapy for the treatment of relapsed or refractory multiple myeloma in adults who have received one to three prior lines of therapy. The regimen consists of carfilzomib (a proteasome inhibitor), daratumumab (a monoclonal antibody targeting CD38 on myeloma cells), and dexamethasone (a corticosteroid). Carfilzomib inhibits the proteasome, leading to accumulation of misfolded proteins and apoptosis in cancer cells. Daratumumab targets CD38 on the surface of myeloma cells, inducing cell death through direct cytotoxicity and immune-mediated mechanisms such as antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity. Dexamethasone provides anti-inflammatory and immunosuppressive effects[1][2][6].
