Overview

Carfilzomib + IMiD refers to the combination of carfilzomib, a second-generation proteasome inhibitor, with an immunomodulatory drug (IMiD) such as lenalidomide or thalidomide. Carfilzomib is used primarily in the treatment of multiple myeloma, particularly in patients who have relapsed or are refractory to previous therapies. It works by selectively inhibiting the chymotrypsin-like activity of the 20S proteasome, leading to accumulation of proteins within myeloma cells and subsequent cell death. When combined with an IMiD—agents that modulate immune response and have direct anti-myeloma effects—the regimen has demonstrated deep responses and improved survival outcomes in both newly diagnosed and relapsed/refractory multiple myeloma settings[1][2][6]. The most studied combination is carfilzomib with lenalidomide and dexamethasone (KRd), which forms a backbone for frontline therapy strategies.
