The combination of carfilzomib, thalidomide, and dexamethasone (often abbreviated as KTd) is a multi-agent chemotherapy regimen primarily used for the treatment of multiple myeloma, including both newly diagnosed and relapsed/refractory cases. Carfilzomib is a second-generation proteasome inhibitor that irreversibly binds to the constitutive proteasome and immunoproteasome, leading to cell cycle arrest and apoptosis in malignant plasma cells. Thalidomide is an immunomodulatory agent with anti-angiogenic properties. Dexamethasone is a corticosteroid that enhances the cytotoxic effects of other agents in the regimen. This combination has demonstrated efficacy as induction and consolidation therapy in transplant-eligible patients with multiple myeloma, showing high response rates and manageable toxicity profiles[1][2][3][8].
