Overview

Carfilzomib Rompharm is a generic version of the proteasome inhibitor carfilzomib, developed by the Romanian pharmaceutical company Rompharm. As a second-generation selective proteasome inhibitor, it is a tetrapeptide epoxyketone that irreversibly binds to the N-terminal threonine-active sites of the 20S proteasome, which is the proteolytic core of the 26S proteasome. Inhibition of the proteasome prevents the degradation of polyubiquitinated proteins, leading to an accumulation of misfolded proteins and the induction of the unfolded protein response (UPR). This process ultimately triggers cell cycle arrest and apoptosis, particularly in malignant plasma cells which are highly sensitive to proteasome inhibition. Carfilzomib Rompharm is approved for the treatment of patients with multiple myeloma, typically in relapsed or refractory settings.
