Overview

Carisbamate is an investigational small molecule anticonvulsant developed primarily for the treatment of epilepsy, including drug-resistant focal epilepsy and Lennox-Gastaut syndrome. It acts mainly by inhibiting voltage-gated sodium channels, particularly Nav1.2, and may also block T-type calcium channels, stabilizing neuronal activity and reducing abnormal firing that leads to seizures[1][6][7][8]. Carisbamate has demonstrated efficacy as adjunctive therapy in reducing seizure frequency in clinical trials but has not received full marketing approval. The drug was developed by Johnson & Johnson Pharmaceutical Research and Development under the brand name Comfyde[7]. Its mechanism of action is not fully understood but involves modulation of neuronal excitability through sodium channel inhibition[6][8].
