Overview

Carmoterol is an investigational ultra-long-acting β2-adrenoceptor agonist (ultra-LABA) developed for the treatment of asthma, chronic obstructive pulmonary disease (COPD), and chronic bronchitis. It acts as a highly selective and potent bronchodilator with a rapid onset of action and duration exceeding 24 hours after inhalation. Preclinical studies demonstrated that carmoterol has greater potency than formoterol and salmeterol, with over 100-fold selectivity for bronchial muscle compared to myocardial tissue. Its mechanism involves activation of the β2-adrenoceptor on airway smooth muscle, leading to relaxation and bronchodilation. Despite promising early results, clinical development was discontinued around 2010 due to lack of competitive advantage[1][2][3][7].
