Overview

This combination regimen is a high-dose chemotherapy protocol followed by autologous peripheral blood stem cell transplantation (ASCT) and consolidation therapy. Investigated by the University of Maryland, Baltimore, the protocol was designed for patients with poor-prognosis low-grade non-Hodgkin lymphoma (NHL) and chronic lymphocytic leukemia (CLL). The treatment sequence involves an intensive conditioning phase using carmustine (BCNU) and melphalan to achieve bone marrow ablation, followed by stem cell support. This is followed by a consolidation phase consisting of the DCEP regimen (dexamethasone, cyclophosphamide, etoposide, and cisplatin) supplemented with paclitaxel and additional cisplatin. Paclitaxel, a taxane, acts by stabilizing microtubules and inhibiting cell division, while the other components provide synergistic DNA-damaging and pro-apoptotic effects to eradicate residual malignant cells and improve long-term outcomes in high-risk patients.
