Overview

This high-dose chemotherapy (HDC) regimen, developed and evaluated by the Instituto Nacional de Ciencias Medicas y Nutricion Salvador Zubiran (INCMNSZ), is an intensive adjuvant treatment strategy for patients with triple-negative breast cancer (TNBC) who have residual disease (non-pathologic complete response) following standard neoadjuvant chemotherapy. The regimen consists of a combination of potent alkylating agents—carmustine, cyclophosphamide, carboplatin, and busulfan—administered at high doses to overcome chemoresistance and eradicate micrometastatic disease. This intensive chemotherapy is followed by autologous hematopoietic stem cell transplantation (HSCT) to restore bone marrow function. The approach targets the inherent genomic instability and DNA repair deficiencies often found in TNBC, aiming to improve disease-free and overall survival in this high-risk patient population.
