Overview

Carmustine + ifosfamide is a combination of two alkylating chemotherapeutic agents used together in oncology regimens for the treatment of various malignancies. Carmustine (a nitrosourea) and ifosfamide (an oxazaphosphorine) both act by alkylating DNA, leading to cross-linking and inhibition of DNA replication and transcription, ultimately resulting in cell death. This combination may be used as part of high-dose chemotherapy protocols or salvage regimens for cancers such as lymphomas, brain tumors, sarcomas, and other solid tumors. Each drug has a distinct toxicity profile; carmustine is notable for pulmonary toxicity and delayed myelosuppression, while ifosfamide can cause hemorrhagic cystitis and neurotoxicity. The rationale for combining these agents lies in their different mechanisms within the alkylator class and non-overlapping toxicities.
