Overview

Carmustine + thalidomide is an experimental combination therapy consisting of two small molecule drugs, carmustine and thalidomide. Carmustine is a cell-cycle nonspecific alkylating agent that exerts its antitumor effects by cross-linking DNA and RNA, leading to inhibition of DNA replication and transcription, ultimately resulting in cell death[2]. Thalidomide is an immunomodulatory agent with anti-angiogenic properties; it modulates the immune response by inhibiting the release of inflammatory cytokines such as tumor necrosis factor-alpha (TNF-α) and affects angiogenesis through multiple mechanisms[1][6]. The combination has been studied preclinically for enhanced antiproliferative effects against malignant glioma, where it demonstrated increased inhibition of tumor growth compared to either drug alone[5]. Both agents are individually approved for use in various cancers including multiple myeloma, but this specific combination does not have established regulatory approval.
