Overview

**Carmustine implant** is a locally delivered, sustained-release intracranial chemotherapy product marketed as **Gliadel Wafer**. Each biodegradable polifeprosan 20 wafer contains 7.7 mg of the nitrosourea alkylating agent carmustine and is placed in the resection cavity following surgical removal of a brain tumor. The polymer matrix gradually degrades, delivering carmustine directly at the tumor site while limiting systemic exposure and bypassing the blood-brain barrier. It is FDA-approved as an adjunct to surgery and radiation for newly diagnosed high-grade glioma and as an adjunct to surgery for recurrent glioblastoma.
