Overview

Carnosinol is a small molecule derivative of L-carnosine designed to act as a potent scavenger of reactive carbonyl species (RCS). Unlike its parent compound, carnosinol is modified at the carboxyl group to resist degradation by carnosinase enzymes and exhibit high oral bioavailability. Its primary mechanism involves the sequestration of α,β-unsaturated aldehydes, such as 4-hydroxynonenal (HNE), acrolein (ACR), and methylglyoxal (MG), which are key mediators of oxidative stress and tissue damage in metabolic and inflammatory diseases. In preclinical models, carnosinol has shown efficacy in mitigating diet-induced obesity, insulin resistance, dyslipidemia, and non-alcoholic steatohepatitis (NASH). Additionally, it has been investigated for its ability to reduce the progression of hyperglycemia-induced invasive pancreatic cancer by mitigating systemic and tumour carbonyl stress.
