Carperitide is a recombinant form of human alpha atrial natriuretic peptide (ANP), a peptide hormone with potent diuretic, natriuretic, and vasodilatory effects. It is primarily used in Japan for the treatment of acute decompensated heart failure and has also been studied for its ability to reduce postoperative atrial fibrillation after cardiac surgery. Carperitide acts as an agonist at the atrial natriuretic peptide receptor NPR1, leading to increased production of cyclic guanosine monophosphate (cGMP). This signaling cascade results in vasodilation, inhibition of the renin–angiotensin–aldosterone system (RAAS), and reduction in cardiac preload and afterload. The drug may also exert anti-ischemic effects and protect against myocardial cell membrane damage[2][4][5][6][7].
