Carphenazine is a phenothiazine antipsychotic agent with a piperazine side-chain, used primarily in the treatment of acute or chronic schizophrenia, especially in hospitalized patients. It acts by blocking postsynaptic mesolimbic dopaminergic D1 and D2 receptors in the brain, leading to antipsychotic effects. Carphenazine also depresses the release of hypothalamic and hypophyseal hormones and is believed to affect basal metabolism, body temperature, wakefulness, vasomotor tone, and emesis through depression of the reticular activating system. The drug belongs to the largest class of neuroleptic antipsychotics (the phenothiazines) and has been associated with side effects such as akathisia, tardive dyskinesia, and extrapyramidal symptoms. Carphenazine was first approved in 1982 but has since been withdrawn from many markets[1][2][3][4].
