CART-meso is an investigational autologous chimeric antigen receptor (CAR) T-cell therapy engineered to target mesothelin, a cell surface protein highly expressed in several solid tumors including malignant pleural mesothelioma, ovarian carcinoma, and pancreatic ductal adenocarcinoma. The therapy involves collecting a patient's own T cells via apheresis and genetically modifying them using lentiviral transduction to express a CAR construct. This construct contains an anti-mesothelin single-chain variable fragment (derived from the mouse monoclonal antibody SS1), fused with intracellular signaling domains 4-1BB and CD3zeta. Once infused back into the patient, these modified T cells are designed to recognize and kill tumor cells expressing mesothelin. Early-phase clinical studies have shown that CART-meso is generally well tolerated with limited clinical activity; ongoing research aims to improve efficacy by developing fully human anti-mesothelin CARs.
