CART123 + cyclophosphamide + fludarabine is a combination therapy consisting of a chimeric antigen receptor T cell (CAR-T) product targeting CD123 (CART123), combined with the chemotherapeutic agents cyclophosphamide and fludarabine. - **CART123** is an autologous CAR-T cell therapy engineered to recognize and eliminate cells expressing CD123, which is commonly overexpressed in certain hematologic malignancies such as acute myeloid leukemia (AML). The mechanism involves genetically modifying patient T cells to express a synthetic receptor that binds specifically to CD123 on malignant cells, leading to targeted cytotoxicity. - **Cyclophosphamide** and **fludarabine** are used as lymphodepleting chemotherapy prior to CAR-T infusion. Cyclophosphamide is an alkylating agent that crosslinks DNA, while fludarabine is a purine analog that inhibits DNA synthesis; both suppress the immune system and enhance CAR-T cell expansion and persistence by reducing regulatory immune elements. This combination aims to maximize anti-tumor efficacy by first depleting endogenous lymphocytes with chemotherapy (cyclophosphamide/fludarabine), then infusing CART123 for targeted immunotherapy against CD123-positive cancer cells.
