CART45RA NKG2D cells is an investigational, donor-derived (allogeneic) CAR T-cell therapy currently in Phase I clinical development for the treatment of advanced sarcoma and relapsed/refractory T-cell acute lymphoblastic leukemia (T-ALL). Developed by Dr. Antonio Pérez Martínez at the Hospital Universitario La Paz (IdiPAZ) in Spain, the therapy utilizes CD45RA+ memory T cells transduced with a lentiviral vector to express a chimeric antigen receptor (CAR) based on the natural killer group 2, member D (NKG2D) receptor. This allows the engineered T cells to recognize and eliminate tumor cells expressing NKG2D ligands (NKG2DL), such as MICA, MICB, and ULBP1-6, which are frequently upregulated in sarcomas and other malignancies. The therapy is being evaluated in clinical trials (including the CAR4SAR trial, NCT06087341) for safety, feasibility, and persistence, utilizing both systemic (intravenous) and local (intratumoral) routes of administration.
