CARTemis-1-BB is an autologous chimeric antigen receptor (CAR) T-cell therapy targeting B-cell maturation antigen (BCMA), specifically engineered with a 4-1BB (CD137) costimulatory domain. Developed by researchers at the Instituto de Biomedicina de Sevilla (IBIS), it is designed for the treatment of multiple myeloma. Compared to CD28-containing counterparts (such as CARTemis-1-28), CARTemis-1-BB demonstrates superior persistence and metabolic fitness, characterized by higher maximal respiration and spare respiratory capacity following antigen encounter. The manufacturing process involves an 8-day expansion period using anti-CD3/CD28 beads and IL-7/IL-15. Preclinical and quality assessment studies indicate that the 4-1BB domain promotes a metabolic profile conducive to sustained cytotoxic activity over multiple antigen encounters.
