Overview

Cassia fistula syrup is a herbal medicinal product derived from the fruit pulp of the *Cassia fistula* L. tree, commonly known as the golden shower tree. It is a staple of traditional Persian Medicine (where it is known as Foloos) and Ayurvedic medicine, primarily indicated for the treatment of chronic and functional constipation. The syrup's therapeutic effect is attributed to its high content of anthraquinone glycosides, such as sennosides and rhein. Upon ingestion, these compounds are metabolized by colonic microflora into active forms that stimulate the myenteric plexus, thereby increasing intestinal motility and peristalsis. Additionally, they inhibit water reabsorption by modulating aquaporin-3 expression and promote the secretion of electrolytes and fluid into the bowel lumen. Clinical trials have demonstrated its efficacy and safety profile in sensitive populations, including children and patients with chronic kidney disease, often showing non-inferiority to osmotic laxatives like lactulose.
