Overview

Cathflo Activase (alteplase) is a recombinant tissue plasminogen activator (tPA) biologic that acts as a fibrinolytic enzyme. It works by binding to fibrin and converting plasminogen to plasmin, which subsequently degrades fibrin into soluble products. Developed by Genentech, Cathflo Activase is specifically a 2 mg formulation of alteplase FDA-approved for the restoration of function to occluded central venous access devices. Memorial Healthcare System is currently investigating its off-label use in a Phase 4 clinical trial for patients with recurrent pleural effusion. In this context, the drug is administered intrapleurally in combination with talc slurry pleurodesis to break down pleural loculations and improve radiographic outcomes and treatment success.
