CB4209 is a mitochondria-derived peptide analog of MOTS-c (mitochondrial open reading frame of the 12S rRNA-c) developed by CohBar as a first-in-class investigational therapy targeting energy regulation and lipid metabolism for nonalcoholic steatohepatitis (NASH), nonalcoholic fatty liver disease (NAFLD), obesity, and related metabolic disorders.[7][9] As a MOTS-c analog, CB4209 is designed to enhance metabolic flexibility and insulin sensitivity, reduce liver fat and triglyceride accumulation, and induce selective fat mass loss, effects that have been demonstrated preclinically in diet-induced obesity and NASH mouse models where CB4209 reduced NAFLD activity scores and improved NASH histology.[1][4][5][6][11] The agent is currently characterized as a preclinical mitochondrial peptide–based epigenetic/metabolic modulator, with no reported human clinical trial data distinct from its closely related analog CB4211.[3][7][9]
