CBT300 is a first-in-class biologic fusion protein developed by Creative BioTherapeutics (CBT) to target extracellular Glucose-Regulated Protein 78 (ecGRP78), a key survival mechanism used by cancer cells to facilitate drug resistance and immune evasion. The molecule consists of the GRP78 binding domain from Receptor Tyrosine Kinase Orphan Receptor-1 (ROR1) fused to a human Fc domain. By inhibiting ecGRP78, CBT300 strips away protective oncofetal proteins (such as ROR1 and Cripto) and the checkpoint protein PD-L1 from the surface of tumor cells, thereby reversing resistance to standard therapies and enhancing immune recognition. Currently in preclinical development, CBT300 has demonstrated efficacy in various models, including metastatic breast cancer, glioblastoma, lung, colon, melanoma, ovarian, and renal cancers, and is intended for use as both a monotherapy and a combination treatment to enhance current therapies at lower doses.
