CCA16-DOTAY is a clearing agent (CA) used as a critical component in DOTA-based Pretargeted Radioimmunotherapy (DOTA-PRIT) regimens. It is a 500-kDa aminodextran polymer scaffold conjugated to approximately 16 DOTA-Yttrium (stable Y-89) complexes. In the PRIT workflow, CCA16-DOTAY is administered intravenously after a bispecific antibody (BsAb) has localized to the tumor but before the administration of a radiolabeled hapten (such as [177Lu]Lu-DOTA-Bn or [225Ac]PrDOTA). Its primary function is to bind to the circulating, unbound BsAb and facilitate its rapid hepatic clearance. By removing the BsAb from the blood pool, CCA16-DOTAY prevents the radiolabeled hapten from binding to circulating antibody, thereby reducing off-target radiation exposure and significantly improving the tumor-to-background targeting ratio. This agent has been extensively utilized in preclinical mouse xenograft models for cancers expressing HER2 (breast, gastric, and ovarian) and GPA33 (colorectal).
