CD117-CAR-iNKT cells are a form of adoptive cell therapy in which invariant natural killer T (iNKT) cells are genetically engineered to express a chimeric antigen receptor (CAR) targeting CD117, also known as the KIT receptor. The primary mechanism involves redirecting the cytotoxic activity of iNKT cells specifically against tumor or malignant hematopoietic stem/progenitor cells that express CD117. This approach is being investigated for advanced systemic mastocytosis and acute myeloid leukemia (AML), where the KIT D816V mutation is common. Preclinical studies have shown that these engineered iNKT or T-cells can effectively kill KIT D816V-positive tumor and patient-derived mast cell lines in vitro and reduce tumor burden in vivo, with increased survival observed in animal models. However, there is concern about potential collateral damage to healthy hematopoietic stem/progenitor populations due to their expression of CD117[3][1].
