CD117 CAR-T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target CD117 (also known as KIT or c-Kit), a receptor tyrosine kinase expressed on hematopoietic stem and progenitor cells (HSPCs), acute myeloid leukemia (AML) cells, and neoplastic mast cells. This cell-based biologic involves genetically modifying autologous or engineered T cells to express a CAR directed against CD117. Due to the expression of CD117 on both malignant cells and healthy HSPCs, the therapy is being explored for dual utility: as a direct treatment for AML and myelodysplastic syndrome (MDS), and as a non-genotoxic conditioning regimen to deplete host HSPCs prior to allogeneic hematopoietic stem cell transplantation (alloHSCT). Some constructs incorporate safety switches, such as inducible Caspase 9 (iC9), to allow for the termination of CAR-T activity and prevent long-term myelosuppression. Research is also exploring off-the-shelf variants using invariant natural killer T (iNKT) cells to mitigate graft-versus-host disease (GVHD) and enhance antitumor efficacy.
