CD123-2B4.zeta chimeric antigen receptor natural killer cells is an investigational cell-based immunotherapy designed for the treatment of acute myeloid leukemia (AML). This biologic therapy consists of allogeneic natural killer (NK) cells, typically derived from healthy donor peripheral blood, which are genetically engineered to express a chimeric antigen receptor (CAR) specific to CD123 (interleukin-3 receptor alpha subunit). CD123 is a well-validated target in AML, as it is overexpressed on malignant blasts and leukemic stem cells compared to healthy hematopoietic cells. The CAR construct is specifically tailored for NK cell biology, incorporating the 2B4 (CD244) co-stimulatory domain and the CD3-zeta signaling domain to enhance cytotoxic responses. To address the challenge of limited NK cell persistence, these cells are often further modified to co-express secretory interleukin-15 (IL-15), providing autocrine support for cell survival and proliferation. This "off-the-shelf" approach aims to provide a potent anti-leukemic effect with a lower risk of graft-versus-host disease compared to T-cell-based therapies.
