CD123-4-1BB zeta CAR-NK cells are an experimental adoptive cell therapy consisting of primary human natural killer (NK) cells engineered to express a chimeric antigen receptor (CAR) targeting CD123 (interleukin-3 receptor alpha), an antigen highly expressed on acute myeloid leukemia (AML) cells and leukemic stem cells. The CAR construct utilizes a CD123-specific single-chain variable fragment (scFv) linked to a 4-1BB (CD137) costimulatory domain and a CD3-zeta signaling domain. This therapy aims to redirect NK cell cytotoxicity specifically toward AML cells while leveraging the innate safety profile of NK cells compared to T cells. Research indicates these cells exhibit antigen-specific anti-leukemic activity in vitro and in vivo, though persistence may be limited without additional cytokine support like IL-15.
