CD123-CODV-TCE (also known as SAR440340) is a bispecific T-cell engager (TCE) developed by Sanofi for the treatment of acute myeloid leukemia (AML). It utilizes Sanofi's proprietary Cross-Over-Dual-Variable-Domain (CODV) format, which is engineered onto a humanized IgG1 antibody backbone with a silenced Fc region to minimize non-specific T-cell activation and cytokine release. The molecule is designed to simultaneously bind to CD3 on T cells and CD123 (the alpha chain of the interleukin-3 receptor) on leukemic blasts and leukemic stem cells (LSCs). This dual binding facilitates the formation of an immunological synapse, leading to T-cell activation and the subsequent release of cytotoxic granules, such as granzyme B and perforin, to induce the lysis of CD123-expressing cancer cells. While it demonstrated potent preclinical activity and specific killing of primary AML cells regardless of antigen density, Sanofi terminated the clinical development of the SAR440340 program in early 2022.
