CD123 ENG iM T cells are an investigational cell-based immunotherapy consisting of T cells genetically engineered to express two primary components: a CD123-specific bispecific T-cell engager (ENG) and an inducible costimulatory molecule composed of MyD88 (iM). The ENG molecule is a bispecific antibody fragment secreted by the engineered T cells that targets CD123 (interleukin-3 receptor alpha), which is highly expressed on acute myeloid leukemia (AML) blasts and leukemia stem cells, and CD3 on T cells. This allows the engineered cells to directly kill CD123-positive tumor cells and recruit bystander T cells to the tumor site. The inducible MyD88 (iM) component is a molecular switch activated by a chemical inducer of dimerization (CID), such as rimiducid (AP1903). Upon activation, the iM domain triggers toll-like receptor (TLR) signaling pathways to enhance T-cell expansion, persistence, and sequential killing capacity, particularly within the immunosuppressive microenvironment of AML.
