CD123.4-1BBz CAR T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target CD123 (interleukin-3 receptor alpha chain), which is highly expressed on acute myeloid leukemia (AML) blasts and leukemic stem cells. The construct utilizes a 4-1BB (CD137) costimulatory domain linked to a CD3-zeta signaling endodomain to enhance T-cell activation, expansion, and persistence upon antigen engagement. Developed at the Baylor College of Medicine, this specific CAR-T construct has been evaluated in preclinical studies, often in comparison with CD28-based endodomains or as part of combinatorial targeting strategies (e.g., with CD33 or CLL-1) to overcome antigen heterogeneity and prevent relapse in AML.
