CD123.ENG.iMC T cells are an investigational cell-based immunotherapy being developed for the treatment of acute myeloid leukemia (AML). These T cells are genetically engineered using a dual-component system: a CD123-specific bispecific T-cell engager (ENG) and an inducible co-stimulatory molecule (iMC) comprising MyD88 and CD40. The ENG component allows the engineered cells to secrete bispecific molecules that bridge T cells (via CD3) and AML cells (via CD123), facilitating the recruitment and activation of both the engineered and bystander T cells against leukemia blasts and stem cells. The iMC component acts as a molecular switch, activated by a small-molecule chemical inducer of dimerization (CID), to provide potent co-stimulation. This design is intended to enhance the expansion, persistence, and effector function of the T cells, addressing common limitations of CAR-T therapies such as exhaustion during chronic antigen exposure.
