CD123CAR-41BB-CD3zeta-EGFRt is an investigational donor-derived **CD123-directed CAR-T cell therapy** for relapsed acute myeloid leukemia after allogeneic hematopoietic stem-cell transplantation. Patient donor T cells are genetically engineered to express a second-generation chimeric antigen receptor recognizing CD123, also known as interleukin-3 receptor alpha, on AML blasts. The construct incorporates intracellular 4-1BB costimulatory and CD3-zeta activation domains to promote CAR-T-cell activation, expansion, and cytotoxic killing of CD123-positive leukemia cells. It also expresses truncated epidermal growth factor receptor as a selectable and safety marker that can permit depletion of infused CAR-T cells with cetuximab if clinically required. The therapy was evaluated in a Phase 1 study as a single intravenous infusion following lymphodepleting preconditioning.
