Overview

CD123xCD3 ENG-T is an adoptive cellular immunotherapy consisting of T cells genetically engineered to secrete a bispecific T-cell engager (ENG) protein targeting CD123 and CD3. Developed primarily for the treatment of acute myeloid leukemia (AML), these "ENG-T" cells function by producing a protein that bridges CD123-positive leukemic blasts with the CD3 complex on T cells, thereby inducing T-cell activation and tumor cell lysis. Unlike traditional CAR-T cells which express a membrane-bound receptor, ENG-T cells secrete the engager molecule, allowing them to recruit and activate both the engineered cells and endogenous, non-modified bystander T cells within the tumor microenvironment. Research indicates that pre-selection of CD4+ and CD8+ T-cell subsets during manufacturing improves product expansion, persistence, and anti-tumor efficacy compared to unselected products by reducing exposure to inflammatory cytokines and preventing phenotypic imprinting by contaminating myeloid cells.
