CD133 CAR-T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target CD133 (Prominin-1), a glycoprotein expressed on the surface of cancer stem cells and brain tumor-initiating cells (BTICs). Developed by researchers at McMaster University and the University of Toronto, this therapy utilizes a second-generation CAR construct incorporating a single-chain variable fragment (scFv) derived from the RW03 monoclonal antibody, linked to CD28 costimulatory and CD3ζ signaling domains. The therapy is intended to target and eliminate the CD133-positive cell populations that are often resistant to conventional chemotherapy and radiotherapy, thereby preventing tumor recurrence. Preclinical studies in glioblastoma models showed significant reductions in tumor burden and improved survival, supporting its advancement into Phase 1 clinical trials for patients with recurrent glioblastoma.
