CD19-BBz CAR T cells are a second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target the CD19 antigen, a protein expressed on the surface of most B-cell malignancies. The "BBz" designation refers to the intracellular signaling domains: the 4-1BB (CD137) costimulatory domain and the CD3ζ (zeta) activation domain. This specific architecture is characterized by enhanced T-cell persistence and a distinct metabolic profile compared to CD28-based CARs, making it a foundational design in cellular immunotherapy. It is the basis for several approved therapies, including tisagenlecleucel, and is widely used in clinical trials for B-cell acute lymphoblastic leukemia (B-ALL) and various B-cell lymphomas.
