CD19-CAR-DNT cells (specifically the RJMty19 program) represent an innovative approach to chimeric antigen receptor (CAR) therapy for autoimmune diseases. This modality utilizes double-negative T cells (DNTs)—a unique subset of mature T lymphocytes defined by the CD3+CD4-CD8- phenotype—engineered to express a CAR targeting the CD19 antigen. Unlike conventional CD4+ or CD8+ T cells, DNTs possess innate-like properties and can exert potent cytotoxicity without being restricted by major histocompatibility complex (MHC) molecules. Crucially, DNTs are associated with a significantly lower risk of inducing graft-versus-host disease (GvHD), which facilitates their potential use as an allogeneic, "off-the-shelf" therapeutic. In the context of relapsed or refractory autoimmune conditions such as systemic lupus erythematosus (SLE), ANCA-associated vasculitis, and systemic sclerosis, these cells are designed to achieve deep and durable depletion of pathogenic B cells, thereby resetting the immune system and potentially inducing long-term remission.
