CD19 CAR-gamma-delta T cells are an allogeneic, "off-the-shelf" cell therapy consisting of gamma-delta (γδ) T cells genetically engineered to express a chimeric antigen receptor (CAR) targeting the CD19 antigen. Unlike conventional alpha-beta (αβ) CAR-T cells, γδ T cells recognize targets in an MHC-independent manner, which significantly reduces the risk of graft-versus-host disease (GvHD) and eliminates the need for patient-specific HLA matching. This therapy utilizes a dual-mechanism of action: the CAR provides specific recognition of CD19-positive B-cells, while the endogenous γδ T-cell receptor (TCR) provides innate anti-tumor activity against a broader range of stress-induced ligands on tumor cells. This modality is primarily developed for B-cell malignancies, such as non-Hodgkin lymphoma (NHL) and acute lymphoblastic leukemia (ALL), and has received FDA Fast Track designation for certain advanced lymphoma indications.
