Drug intelligence / Profile preview
CD19 CAR T cells (CD28-costimulated) represent a class of second-generation chimeric antigen receptor (CAR) T-cell therapies engineered to target the CD19 antigen, which is widely expressed on B-cell lineage cells and B-cell malignancies. These CAR T cells incorporate a CD28 costimulatory domain alongside the CD3-zeta signaling domain. The inclusion of the CD28 costimulatory domain provides rapid and robust T-cell activation, proliferation, and cytolytic activity, although it is typically associated with shorter in vivo persistence compared to 4-1BB-costimulated CAR T cells. This platform is utilized in several clinical-stage and approved autologous CAR T-cell therapies, such as axicabtagene ciloleucel and brexucabtagene autoleucel, as well as various academic point-of-care programs (e.g., at Sheba Medical Center) for the treatment of relapsed or refractory B-cell malignancies, including B-cell acute lymphoblastic leukemia (B-ALL) and various B-cell non-Hodgkin lymphomas (B-NHL).
