Overview

CD19-CD133 bispecific CAR-T is a preclinical chimeric antigen receptor T-cell (CAR-T) therapy that simultaneously targets two antigens: CD19, a pan-B-cell surface antigen, and CD133 (also known as PROM1), a stem cell marker that is directly regulated by KMT2A-fusion oncoproteins. This dual-targeting approach is designed to enhance antileukemic activity and reduce the risk of immune escape in high-risk KMT2A-rearranged (KMT2Ar) acute leukemias, which typically express both markers. Research presented at ASH 2025 indicates that while this construct is effective, CAR-iNKT cells using the same bispecific CAR may offer superior trafficking and retention in the leptomeningeal space compared to standard CAR-T cells due to enhanced VLA-4-mediated adhesion.
