CD19-targeted CAR T cell therapy is a form of adoptive cell immunotherapy where a patient's T cells are genetically engineered to express a chimeric antigen receptor (CAR) that specifically recognizes the CD19 antigen. CD19 is a transmembrane protein widely expressed throughout B-cell development and on the majority of B-cell malignancies, making it an ideal target for treating B-cell lymphomas and leukemias. The therapy involves collecting T cells via leukapheresis, modifying them ex vivo (typically using viral vectors) to express the CAR, and re-infusing them into the patient following lymphodepleting chemotherapy. Once infused, these cells expand and target CD19-positive cells, leading to cytotoxic cell death. Several CD19-targeted CAR T products have received regulatory approval, including tisagenlecleucel, axicabtagene ciloleucel, and lisocabtagene maraleucel, for indications such as diffuse large B-cell lymphoma (DLBCL), follicular lymphoma, and acute lymphoblastic leukemia (ALL).
