CD19CAR-CD28-CD3zeta-EGFRt-expressing naive and memory T-lymphocytes is an investigational autologous chimeric antigen receptor (CAR) T-cell therapy developed by City of Hope Medical Center. The therapy involves the genetic modification of a patient's own naive and memory T-cells (Tn/mem, encompassing naive, central memory, and stem cell memory T-cells) using a lentiviral vector to express a CAR that targets the CD19 antigen, a protein expressed across B-cell lineage malignancies. The CAR construct includes a CD28 costimulatory domain fused to a CD3 zeta signaling domain to enhance T-cell activation and persistence. Additionally, the cells express a truncated human epidermal growth factor receptor (EGFRt), which serves as both a tracking marker and a safety mechanism, allowing for the elimination of the CAR T-cells via cetuximab-mediated antibody-dependent cellular cytotoxicity if necessary. This therapy is primarily being evaluated for the treatment of primary or secondary central nervous system (CNS) lymphoma, where it is administered directly into the cerebrospinal fluid via intracerebroventricular (ICV) injection to bypass the blood-brain barrier.
